They work through a low-noise air treating system, which inlets laminar air in the room via HEPA filters. Air will be then aspired by the grids on the floor.
The purity of air in these rooms is definitely higher than in a surgery. Hence, operators working within them need to be properly trained and wear sterile gowns, shoe covers, caps and masks.
